The home assessment
A free visit of about an hour to plan the care week with you.
What happens at the assessment
About an hour at home, free of charge (sample).

1
A conversation
About the day: what is easy, what is getting harder, and what the person wants to keep doing themselves.
2
A look around the house
Stairs, bathroom, kitchen and the way in, so carers know where things are and what could be easier.
3
The week, drafted
The coordinator sketches the visits on a blank week with you: which days, which times, how long.
4
A written plan and price
Sent within two working days (sample). Nothing starts until you agree.
What to have ready
- The list of medicines, or the packs themselves
- Names and numbers of the doctor and close family
- Hospital discharge papers, if there was a recent stay
- Any routines that matter: when to get up, favourite meals, the dog's walk
Every care week starts empty
A care coordinator visits you at home, free of charge, to talk through the day and plan the visits with you. There is nothing to sign at the assessment.
Your week, before the assessment
At the home assessment we fill it in with you: which days, which times, how long.
